Developmental Disability Services Eligibility Process
Do you or someone you know need to apply for Developmental Disability Services, but don’t know where to start?
You are not alone, so let’s see if we can demystify the process together.
In order for Jackson County DD Services (JCDDS) to fully determine eligibility for services, they will need all of the following information from you:
1. A completed application
2. Copies of any psychological evaluations and IQ (Intelligence quotient) tests that the applicant has completed. Examples of common IQ tests are: the WAIS, the WISC, and the Stanford-Binet.
3. Copies of any adaptive assessments that the applicant has had. Examples of commong assessments: Vineland & ABAS-3
4. A written diagnosis of a developmental disability signed by a physician or psychologist.
*JCDDS does NOT provide evaluations as a part of the application process.

If you don’t have all of your records, that’s OK. Submit what you can, and let an Eligibility Specialist help you with the rest.

Helpful Hints:
The more records you can provide at the time of your application, the faster they will be able to determine your eligibility for services.
If you do not have psychological records on hand, some good sources for obtaining those records may include: school, doctor, the Social Security Administration (if the applicant has been evaluated for Social Security benefits), Vocational Rehabilitation, or mental health clinics.
